How to make a reservation

Reservations can be made through our official line or by phone.
It is possible to come to our clinic without an appointment. We recommend that you make an appointment in advance to ensure a smooth visit.

What to bring on the day

Photo identification
Insurance policy
Vaccine record book, etc.
Photo IDs should be passport (photocopies are acceptable) or Thai driver's license or ID card.

  You will be asked to submit your ID and insurance policy at the reception desk.

  If this is your first visit, you will be asked to fill out a medical questionnaire. If you have insurance, you will be asked to fill out a claim form depending on some insurances.

  Medical examination

  An interpreter will be provided during the consultation, so please consult with us in Japanese.

  Bill

  For cashless by insurance, some policies may require you to sign a claim form at the end.
